If paying in full, a superbill will be provided upon request following the appointment to submit a claim to your medical insurance using out-of-network benefits. To determine how much you will be reimbursed by your insurance, contact your insurance and ask the following questions:
1. Do I have out-of-network benefits? Do I have an out-of-network deductible? How much of my deductible has been met this year?
2. What is my out-of-network co-payment or co-insurance amount? How much can I expect to receive as a reimbursement?
3. Do I need a referral from an in-network provider to see someone out-of-network?
4. How do I submit my claim for reimbursement?
When paying for appointments, refunds are not given if you later intend to use insurance. If appointment is not cancelled within 48-hours of appointment time, no refund is given. Insurance reimbursement is not guaranteed.
