Steps to Becoming a Client
- Ensure your preferred clinician is available.
- If using insurance, please ensure that your insurance plan is in-network. If your insurance plan is listed, please email email@example.com to check whether your preferred clinician is in-network with your plan. If so, we will direct you to the next steps. DO NOT book using the online portal if you are a new client attempting to use insurance.
- If paying cash, you may book an appointment on our scheduling portal after verifying that your preferred clinician is available (see link in Step 1). You must submit the following forms within 48 hours of scheduling to avoid cancellation of your appointment. DO NOT submit these forms unless you have also booked an appointment or they will NOT be reviewed.
